The last five years saw a significant return of epidemic infectious disease, culminating in COVID-19. Detailing his years in the lab developing new vaccines, Hotez also recounts his travels around the world to shape vaccine partnerships with people in countries both rich and poor in an attempt to head off major health problems. Building on the legacy of Dr. Albert Sabin, who developed the oral polio vaccine with Soviet scientists at the height of the Cold War, he explains how he is still working to refresh and redirect vaccine diplomacy toward neglected and newly emerging diseases. Hotez reveals how—during his Obama-era tenure as the US Science Envoy for the Middle East and North Africa, which coincided with both the rise in these geopolitical forces and climate change—he witnessed tropical infectious diseases and established vaccine partnerships that may still combat them up close. He explores why, since 2015, we've seen the decline of global cooperation and cohesion, to the detriment of those programs that are meant to benefit the most vulnerable people in the world. Unfortunately, Hotez asserts, these negative global events kick off a never-ending loop. Problems in a country may lead to disease outbreaks, but those outbreaks can lead to further problems—such as the impact of coronavirus on China's society and economy, which has been felt around the globe. Zeroing in on the sociopolitical and environmental factors that drive our most controversial and pressing global health concerns, Hotez proposes historically proven methods to soothe fraught international relations while preparing us for a safer, healthier future. He hammers home the importance of public engagement to communicate the urgency of embracing science during troubled times. Touching on a range of disease, from leishmaniasis, schistosomiasis, and Middle East Respiratory Syndrome (MERS) to COVID-19, Preventing the Next Pandemic has always been a timely goal, but it will be even more important in a COVID and post-COVID world.

500 years ago, physicians would dress up in what we today would call Halloween costumes when they went to treat victims of plague. Typically the costumes would be something akin to a scary looking bird. The idea was that the costume would both frighten the evil spirits / demons away that were afflicting the patient & also serve as a layer of protection for the physician himself (this is not sexist language as there were no female physicians 500 yrs ago, per-se). For those of us who are of European descent, these were our ancestors; either the people being treated, the physicians or the people cheering on the costumed physicians. These people were not stupid. Rather, they simply didn't know any better & they associated a religious impetus to what ailed the sick rather than a biological impetus.In the present day we had made so many advances in medical science that a time traveler from 500yrs ago would be convinced that we didn't belong to the same planet as them. We have mapped the entire human genome and myriad diseases which were lethal not so long ago have all but been wiped out.Nevertheless, even with all of our modern technology, what the COVID-19 pandemic has made evident is just how LITTLE things have changed in the past 500yrs. On my FACEBOOK page would appear statements such as "No need to wear a mask ~ God has your back." There would also be admonishments against masks by that world renowned epidemiologist named Ted Nugent. I was aghast at how much ignorance & conspiracy theories abounded on the Internet.While our ancestors in Europe were wrong about scary costumes driving off plague, they were on the right track about other things. In 13th century Italy all ships entering Italian ports were to be sequestered up to 40 days to prevent the spread of the Bubonic plague. The Latin word for 40 days? Quarantine. Even though they didn't know what the heck it was, they still used common sense countermeasures.Of course, these days there are politicians who even reject those simple measures. Congressman Jim Jordan spoke in the halls of Congress and asked "What would our founding fathers think of these lockdowns?" Obviously Jordan himself was ignorant of the fact that none of than George Washington forbade his troops from entering Boston in 1776 after an outbreak of smallpox. Good gracious!!Enter the kindly Dr. Peter Hotez. Over the past year he has been thrust into the middle of the maelstrom as a distinguished scientist trying to put out sensible guidance in the midst of chaos & confusion. The present book is a pensive look at how what he calls vaccine diplomacy has worked in the past to eradicate diseases. This has included world actors who cooperated even though they did not particularly like each other. Hotez uses the notable example of this that I was not aware of: collaboration between the United States & the former Soviet Union to wipe out polio. This was something I was not aware of.Hotez takes on a grand tour of the esoteric world of vaccinology & gives us a glimpse into how transnational cooperation is still possible in a broken world. He also is a leader against the ever dangerous anti-vaccination movement which has picked up steam in the US in recent decades. Through it all he maintains his calm demeanor in a world where a physician such as himself is subjected to threats of violence for simply doing his job. Indeed, as a species we have not come so far since the middle ages after all.The people who would benefit the most from this book are people who are in the anti-vaccine community. Unfortunately they're not likely to peruse any books by Hotez as it's not going to tell them what they want to hear. Nevertheless, for anyone interested in facts, medicine or the fruits of cooperation in the medical field which transcends borders, this book is a must-read!
